Monitoring During Labour & Fetal Heart Auscultation

Monitoring During Labour & Fetal Heart Auscultation

Introduction

Intermittent auscultation of the fetal heart rate is an essential practice for monitoring the well-being of the fetus during labor, especially in low-risk pregnancies. Alongside this, regular monitoring of uterine contractions and maternal vital signs ensures the health and safety of both mother and baby. This guide outlines the protocols for intermittent auscultation and other key monitoring practices during labor.

Intermittent Auscultation of Fetal Heart Rate

For Low-Risk Patients:

  1. First Stage of Labor:
  • Frequency: Every 15 minutes.
  • Duration: At least 1 minute after a uterine contraction.
  1. Second Stage of Labor:
  • Frequency: Every 5 minutes.
  • Duration: At least 1 minute after a uterine contraction.

Monitoring Uterine Contractions

  • Frequency: Every 30 minutes during both the first and second stages of labor.

Maternal Vital Signs Monitoring

  1. Temperature:
  • Frequency: Every 4 hours during both the first and second stages of labor.
  1. Blood Pressure:
  • First Stage: Every 4 hours.
  • Second Stage: Every hour.
  1. Pulse:
  • First Stage: Every hour.
  • Second Stage: Every 15 minutes.

Vaginal Examinations

  • First Stage: Every 4 hours, or more frequently if requested by the woman or if there are concerns.
  • Second Stage: Every hour, or more frequently if requested by the woman or if there are concerns.

Urine Monitoring

  • Frequency: Regular checks to ensure the bladder is empty and to monitor for any signs of distress or infection.

Conclusion

Consistent monitoring of the fetal heart rate, uterine contractions, and maternal vital signs is crucial during labor to ensure the well-being of both the mother and baby. Intermittent auscultation is an effective method for low-risk pregnancies, providing necessary information while allowing for mobility and comfort. Always communicate with your healthcare provider to ensure your labor and delivery plan meets your individual needs and circumstances.
